Social Support Group & Centred Based Respite

šŸ¤ What is social support?

Social supportĀ helps older people stay connected, active, and engaged in their community.Ā 

AtĀ St Sergius Aged Care Community Centre in Cabramatta and Blacktown, ourĀ Social Support GroupĀ offers a welcoming, structured program of group-based activities designed to bring joy, connection, and variety to your routine. It’s also a great way to give carers a well-deserved break from time to time..Ā 

šŸŽØ Activities may include:

  • Arts and crafts
  • Gentle exercise
  • Cultural celebrations
  • Games and outings
  • Shared meals and conversation
  • Excursions and events

To join, you’ll need an assessment and approval through theĀ Commonwealth Home Support Programme (CHSP).

When we receive your government assessment, we’ll carefully review it to ensure we can meet your care needs and personal preferences.Ā 

šŸ“‹ Before You Begin Services

OurĀ Care PartnerĀ will invite you to a short interview at ourĀ Community Centre in Cabramatta. Please allow aroundĀ one hourĀ for this meeting. To get started, we’ll ask you to provide the following documents:

Required Documents:

  • My Aged Care Support PlanĀ 
  • Completed Pre-admission form
  • Copy: Power of Attorney/Guardianship Documents
  • Completed CHSP Service Agreement
  • Completed Privacy Agreement

If You Require Medication Support:

  • Medication chart or list from your GP or hospital
  • Completed medical history from your GP (if needed)
  • Webster Pack and any non-packed medications (if applicable)

St Sergius operates in line with theĀ Aged Care Act 2024,Ā Aged Care Principles, and theĀ  Strengthened Aged Care Quality Standards. We also uphold theĀ Statement of Rights, which is available in your preferred language.Ā 

We regularly review our services and work closely with theĀ Aged Care Quality and Safety CommissionĀ to ensure continuous improvement. Our staff, volunteers, and contractors are allĀ police-checked, and we provide ongoing training to maintain the highest standards of care.Ā Ā 


šŸŽ‰ The Social Support Program

Our Social Support Group runsĀ Monday to ThursdayĀ and offers a vibrant, structured program of activities. A schedule is sent to clients in advance.Ā 

What’s Included:

  • 🚐 Return bus transport (for those without transport)
  • ā˜• Morning tea, hot lunch, and afternoon tea
  • šŸŽØ Group activities such as:
  • Art & craft
  • BBQs in the park
  • Bingo & board games
  • Bus outings & group excursions
  • Coffee chats & concerts
  • Cooking & exercise classes
  • Trivia, walking groups, winery visits & more!

šŸ’° Program Cost:

$18.00 per person, per day for Clients with CHSP funding, please refer to Support at Home fees for Clients on Homecare or Support at Home Packages
(Includes all activities, meals, and snacks)
