Your care plan may include a range of different services depending on your needs. In
this guide, we’ve included the rates for our services so that you’ll know what to expect
when you receive your next invoice from us.
As your Approved Provider, we are responsible for providing you with at least one care management activity per month.
Our care management activities include working with you to plan and coordinate your services, making sure your services are working well for you, resolving problems, adjusting services for your changing needs and connecting you to other services and supports. Care management includes time spent on documentation and following up agreed actions.
On a monthly basis, a 10% care management fee will be deducted from your budget and allocated to our care management account.

Delivered by university-qualified and accredited health professionals, our clinical support is grounded in evidence-based practices for prevention, diagnosis, treatment, and ongoing management. You can trust that every service is provided with a strong focus on safety, quality, and personalised care, helping you achieve your health goals with confidence.
At St Sergius, our nursing team is available around the clock to support your health and wellbeing at home. We offer a range of nursing services tailored to the complexity of your care needs, ensuring you receive the right level of support from the right professionals.
Your Care Partner will collaborate with you to ensure your nursing support is matched to your assessed needs and delivered by appropriately credentialed clinicians. Your care plan will be reviewed regularly to ensure it continues to meet your evolving health goals.

Nursing Consumables—What’s Included
The hourly rate for services provided by our Registered Nurses, Enrolled Nurses, and Nursing Assistants includes the cost of standard nursing consumables; items nurses routinely carry, such as bandages, antiseptics, and basic wound care supplies.
For specialised consumables that are tailored to your individual care needs such as prescribed skin emollients for managing skin integrity, specialised dressing or oxygen-related supplies; we’ll help source these and charge them to your care package at cost, ensuring transparency and value.
Your Care Partner will work with you to ensure all consumables are appropriate for your care plan and aligned with your clinical needs.
Our allied health and therapeutic services are designed to help you maintain or improve your physical, functional, and cognitive abilities, so you can continue living safely and independently at home. Whether you're recovering from an illness, wishing to improve your health, or simply looking to stay strong and active, these services offer flexible support tailored to your individual goals, empowering you to live well, with confidence and comfort.

In addition to the services we offer directly, we can support you in accessing services from third-party providers to help meet your broader care needs. The choice of provider is entirely yours; you may select from our panel of trusted partners or nominate a provider of your own. Our providers are thoroughly vetted and monitored to ensure that you receive high quality care.
We’ll work with you to coordinate these services and ensure your chosen provider meets all relevant quality, safety, and administrative standards. Because costs may vary depending on the provider and your individual circumstances, we’ll confirm the price and any applicable contribution before the service is arranged.

Our goal is to make accessing additional support simple, safe, and tailored to your needs.
If you require extra support or more hours of care beyond what your current funding covers, you have the option to purchase additional services at your own expense. These services are charged at the full rates listed in your pricing schedule, and no contribution assessment from Services Australia is required.
The Assistive Technology and Home Modifications (AT-HM) Scheme is designed to help you live safely and independently at home and in your community. It provides separate funding and support under the Support at Home program, with tailored tiers and settings to meet your individual needs.
To ensure safe and effective delivery of assistive technology and home modifications, the following coordination and administration fees apply:
A Co-ordination Fee: Covers the setup, delivery, and training for safe use of equipment and modifications. Capped at 15% of the quoted cost on $1,500 (whichever is lower).
Administration Fee: Covers the management and oversight of your AT-HM services.
Capped at 10% of the quoted cost or $500 (whichever is lower).
Because assistive technology and home modifications are highly individualised, pricing will vary based on your specific needs and circumstances. We will always confirm and agree on the final cost with you before any purchase or installation takes place.
Our Restorative Care pathway is designed to provide early; intensive support aimed at helping you maintain or regain physical and cognitive function and reduce or prevent further decline. This approach combines clinical expertise with personalised care to help you live independently and confidently at home. You may be eligible for assessment under this separate funding pathway, which offers access to additional restorative care support, in additional to your Support at Home funding.
Our clinically qualified Restorative Care Partners will work with you to set up and coordinate your care plan, using a multi-disciplinary approach that includes:
Clinical Care Management is available weekdays from 6am to 6pm and is charged at $190.00 per hour.
Your restorative care plan will be built around individualised goals, with most services delivered by clinical professionals such as nurses and allied health practitioners. These services are charged at the standard rates outlined in your pricing schedule.
Our End-of-Life Care pathway is designed for older individuals with a life expectancy of three months or less who wish to remain in the comfort of their own home. If eligible, you may access this pathway under the Support at Home program.
This pathway provides dedicated funding, separate from other Support at Home classifications. If you transition to End-of-Life Care, it will replace your existing care classification to ensure your support is tailored to your changing needs. If you would like to explore this option, please speak with your Care Partner, who will guide you through the process with sensitivity and care.
While your care pathway will change, your Care Partner will continue to support you, working closely with a Clinical Care Partner to ensure your care is coordinated, compassionate, and responsive.

Personal care
These services are designed to complement existing state and territory-based palliative care programs, ensuring you receive holistic, high-quality support during this important time.
Additional Services refer to any extra support you request that falls outside your government-funded care package. These are paid privately and charged at the full rate listed in the pricing schedule. No contribution assessment from Services Australia is required.
If you choose to access services through a Associate Provider (third-party provider), the final price will include:
You may still be required to contribute to the cost of non-clinical services delivered by third-party providers. Contribution rates are determined by your income and asset assessment conducted by Services Australia.
Your contribution is based on an assessment by the Commonwealth Government via Services Australia, which considers your income and assets.
To avoid a cancellation fee, please provide at least 48 hours’ notice before a scheduled service.
If notice is not received, a cancellation fee for the full service may apply, subject to applicable legislation and unless otherwise agreed.
GST is included in the price where applicable.
If we are able to claim GST on a third-party service, the actual cost to you will be reduced accordingly.
This pricing schedule is valid from 1 November 2025 and may be updated at any time in accordance with the terms of your Service Agreement.
