Assistive Technology & Home Modifications Scheme (AT-HM)

The ATHM scheme is a key component of the Support at Home program. The scheme provides access to equipment and home modifications to enable people to live independently in their homes for longer. The scheme offers different funding tiers to accommodate different needs, up to $15,000 depending on eligibility

You may be able to access:

  • Simple home modifications like ramps, rails and bathroom adjustments.
  • Daily living aids such as shower chairs, personal alarms, jar openers or kettle tippers.
  • Mobility equipment including walkers or wheelchairs.
  • Smart technology like personal alarms to keep you safe and connected.

These products can all help keep you safe by reducing risks and making everyday life easier.

Restorative Care Pathway

The Support At Home Restorative Care Pathway provides intensive support from nursing and/or allied health professionals to support you to build your strength, improve your independence and prevent age related decline. It is a short-term program, lasting 12-16 weeks, with funding up to $6000.

You may be eligible for two periods of restorative care in a 12-month period.

Your plan may include:

  • Physiotherapy or occupational therapy to help you move safely again.
  • Nursing support when you need extra care.

End of Life Pathway

The Support at Home End of Life Pathway supports you or your loved one to remain at home when a person has been diagnosed as having three or less months to live. The pathway provides $25,000 for 12-16 weeks to support you to have additional Support at Home services, complimenting your GP and specialist palliative care team.
